Health Care Plans are developed for students with a health condition that requires monitoring and that requests certain actions in response to specific conditions.  For example,  a student with diabetes may need to test his/her blood glucose in response to "feeling low."  A student with asthma may need to use their medication by inhaler if they experience an asthma attack.
