Application Checklist

This list includes the recommended items to complete the Local Scholarship Application.



  • Citizenship Status (U.S. Citizen, Permanent Resident, AB540)
  • Academic Transcript (GPA & Class Rank)
  • Two References (name, position/relationship, phone number)
  • School & Community Activities
  • School & Community Awards & Honors
  • Employment Information
  • Personal Statement (about 500 words, see below for ideas)


  • Parent/Guardian's Employer
  • Household Size (number of people parents support more than 51%)
  • 2018 Annual Gross Income (estimated)
  • 2018 Untaxed Income (Child Support, Disability, etc.)
  • 2018 Medical Expenses not covered by insurance or Medi-Cal


The personal statement is an essay (about 500 words) that describes the student's academic and career objectives, goals, and aspirations.


Consider including some of the following:

  • What has shaped you as a person?
  • What are your educational and career goals?
  • Why have you chosen these goals?
  • How do you envision making a contribution to your community after completing your education?
  • Have you experienced any special circumstances or unusual hardships that have influenced your choice of college study and/or career path or need for scholarships?


Students are responsible for submitting the first page of their Parent/Guardian's 2018 IRS tax return in order to be eligible for financial need-based scholarships. If a 2018 tax return is not available, students may submit the Parent/Guardian's 2017 tax return.

Tax returns are to be submitted to Cheryl Nickel in the AHS Library. All information is confidential.
